ARTIFICIAL INSEMINATION BETWEEN HUSBAND, WIFE, AND DONOR IDENTITY OF DONOR KNOWN
This Agreement is made and executed on _____________________ (month & day), _________ (year), between________________________________________________ (prospective mother), of __________________________________________ (address), here referred to as Recipient, ______________________________________________ (name of recipient's husband), of ___________________________________________ (address), here referred to as Husband, and ___________________________________ (name of sperm donor), of ________________________________________________ (address), here referred to as Donor.
RECITALS
A. Recipient and her Husband desire to have a child through artificial insemination using sperm donated by Donor. Husband consents to use of semen donated by Donor for purposes of artificial insemination.
B. Donor desires to donate sperm to Recipient and Husband to enable them to have a child through means of artificial insemination.
C. Recipient understands that pregnancy involves risks such as miscarriage, difficult delivery, and birth defects in the child and that such risks apply as well to conception through artificial insemination.
Therefore, the parties agree as follows:
SECTION ONE
MEDICAL EXAMINATION AND TESTING
Prior to donating sperm to Recipient and Husband, Donor agrees to undergo a complete physical and genetic examination to be performed by ___________________________ (name of physician). In addition, Donor agrees to be tested for venereal disease and Human Immunodeficiency Virus (HIV). A copy of the results of Donor's examination and testing shall be provided to Recipient and Husband prior to insemination. Donor agrees to give true and complete answers to all questions asked by the examining physician regarding Donor's health and background. In the event such medical examination and testing reveals Donor has a venereal disease, is HIV positive, or may transmit a genetic defect, Recipient may cancel this Agreement by giving notice of such cancellation to Donor.
SECTION TWO
SPERM DONATION
